Breast reduction is medically indicated when the size and weight of the breasts cause documented physical and psychological symptoms that do not respond to conservative treatment. The following are the most common indicators:

Chronic back and neck pain: Pain that persists despite physical therapy, anti-inflammatory medications, and postural correction. This is the single most common reason patients seek reduction surgery.

Skin irritation and infections: Recurrent fungal infections, intertrigo, and maceration beneath the breast fold — conditions that return despite treatment because the underlying cause (skin-on-skin contact) cannot be eliminated without surgery.

Deep bra strap grooves: Permanent indentations in the shoulders caused by the weight of the breasts. These grooves can compress the brachial plexus nerve, causing numbness, tingling, and weakness in the arms and hands.

Difficulty breathing and exercising: The weight on the chest wall restricts lung expansion, particularly during physical activity and sleep. Many patients avoid exercise entirely — not from lack of motivation, but from physical inability.

The Procedure — What Happens

Breast reduction surgery removes excess breast tissue, fat, and skin to achieve a size proportionate to the patient's body frame. The procedure also repositions the nipple-areola complex to a natural, youthful position.

Surgical techniques:

Wise pattern (anchor incision) — The most versatile technique, suitable for significant reductions. Incision around the areola, vertically down, and along the breast fold. Provides maximum control over shape and symmetry.

Vertical (lollipop) technique — For moderate reductions. Incision around the areola and vertically down only. Less scarring, excellent long-term shape.

Liposuction-assisted reduction — For cases where fat removal alone can achieve the desired result, with minimal scarring.

Duration: 2-4 hours under general anesthesia, depending on the amount of tissue to be removed and the technique used.

Tissue removed: Typically 300-800 grams per breast, though this varies based on the patient's anatomy and goals.

Nipple sensation: In the vast majority of cases, the nipple remains attached to its blood supply and nerve connections via a pedicle. More than 90% of patients retain or regain full sensation.

Breast Reduction Recovery

Most patients take 1-2 weeks off work, though desk jobs can often be resumed within a week. A surgical support bra is worn for 4-6 weeks to support healing and maintain shape. Swelling gradually resolves over 3-6 months, and the final breast shape emerges during this period. Light walking is encouraged from day one; upper body exercise and heavy lifting are restricted for 6 weeks. Scars — while permanent — fade significantly over 12-18 months and are placed within natural skin folds and along the areola border where they become increasingly inconspicuous.

The ability to breastfeed after reduction depends largely on the surgical technique used. Pedicle-based techniques — which keep the nipple connected to the underlying breast tissue — preserve the milk ducts and nerve supply, giving patients the best chance of breastfeeding successfully. If you are planning future pregnancies and breastfeeding is important to you, discuss this with your surgeon before the procedure so the technique can be tailored accordingly. Many patients do breastfeed successfully after reduction, though some may need to supplement.

Temporary changes in nipple and breast skin sensation are common immediately after surgery. This is caused by the stretching and repositioning of nerves during the procedure. In our experience, more than 90% of patients regain full or near-full sensation within 6-12 months as the nerves regenerate. Permanent loss of sensation is rare and is more associated with very large reductions or free nipple graft techniques, which are only used in extreme cases.

Will there be visible scars? Yes, but they fade significantly over 12-18 months. Scars are strategically placed around the areola, vertically below it, and within the natural breast fold — areas that are concealed by bras and swimwear.

Can breasts grow back after reduction? The removed tissue does not regenerate. However, significant weight gain, hormonal changes, or pregnancy can increase breast size. Maintaining a stable weight helps preserve your results.

Is breast reduction covered by insurance? Many insurance plans cover breast reduction when medical necessity is documented — including chronic pain, skin conditions, and nerve compression. We can provide the required documentation to support your claim.

Can reduction be combined with a lift? A breast reduction inherently includes a lift. By removing excess tissue and repositioning the nipple, the procedure achieves both goals simultaneously. A separate lift procedure is not needed.
